Preparing for your first visit

For your first appointment, there is some relevant information that we must record, such as contact details and existing health conditions. We ask new patients to complete a Personal Assessment Form, which we can help you complete.

We do ask that you plan to arrive about ten minutes prior to your scheduled appointment to ensure the form is completed in advance of your appointment time.

Our consultations are not meant to be a one-way street but should be an interactive conversation that addresses any questions or concerns that you may have.  Do write down a list of your symptoms and questions to bring in with you during the appointment….that way, nothing will slip your mind.

Here are some other things that might be helpful to have:

      • Information on any previous surgeries you may have had

      • Any important medical records or reports

      • A pair of worn-in shoes, especially if the problem involves heel pain, sports injuries, or your gait
